Your Initial Visit

Please arrive 15 minutes prior to your appointment in order to complete your medical/dental history forms.  Please bring a list of any current medications you are taking and any dental insurance information.  Parents and/or guardians must accompany all patients under the age of 18.

Your first visit to our office will primarily be for the purpose of gathering information necessary for us to plan your future dental care.

The data collected will include:

  1. Past dental history forms
  2. Medical history
  3. Schedules of any medication being taken
  4. X-Rays (current and any past x-rays forwarded to our office)

Your first visit will be with the Doctor and it is the best time for you to communicate to us any concerns, fears, and expectations which you may have regarding dental health. We are listening.  Since dental treatments must be customized to each patient's unique requirements, we need your input!

After meeting the Doctor, you may be seeing the hygienist on the same day. You should expect some information on the latest techniques of oral hygiene. We will also communicate to you any concerns which we may have concerning your long term "gum" care. A cleaning will be started and completed if possible. As some people require significantly more time to have their teeth cleaned properly, please understand that further scheduling may be required for a thorough cleaning to be done.

Excellence in Dentistry begins with a careful co-diagnosis and treatment plan to move us toward the goals we will establish together. We will perform an extensive and comprehensive examination of the teeth, gums, bite, joints, and soft tissue. When needed, and with your permission, we will utilize all appropriate x-rays, as well as intra-oral photographs and study models of your teeth and jaws to identify areas of concern. We believe this in-depth examination to be the cornerstone of quality dentistry.
